Integrated Grading is assigned based on the scores of weighted grades of Performance Indicators and Process Indicators where the weights (70:30 model) are as follows:

                        Indicators with grade A = 2 (Score = 2 x 4 = 8)

                        Indicators with grade B = 2 (Score = 2 x 3 = 6)

                        Indicators with grade C = 2 (Score = 2 x 2 = 4)

                        Indicators with grade D = 2 (Score = 2 x 1 = 2)

                                    Total Score = 20

                                    No. of Indicators = 8

                                    Performance Score = 20 ¸8 = 2.5

2a. Tours & Inspections Score

      + [Surprise Inspections score (0.3) x 4 or 3 or 2 or 1]
